Clinical Pharmacy Specialist

Getting to know my patients is the highlight of my role at Legacy and enables me to create an effective and reasonable plan of treatment personalized to each patient’s needs. When meeting with patients or providing consults with our geriatricians, I focus on optimizing and decreasing the burden of medications for patients when possible.

Pharmacists are often the most accessible health care clinicians because patients can walk into most pharmacies and request an immediate consultation. Within our senior primary care clinics, we further strengthen the pharmacist-patient partnership by supporting patients during and between clinic visits.

A clinical pharmacy specialist’s care includes reviewing medical histories, considering previous medication success or failure and cost, and ensuring that patients can access their prescribed medications. We also help patients understand their medication regimen and provide disease state and medication education for older adults with diabetes, hypertension, and/or high cholesterol.

My goal for each visit is for patients to leave with new knowledge and a clearer understanding of their medications or chronic diseases. I take the time to meet with patients in person or via telehealth to listen to their concerns and individual health care goals—and to encourage and empower them to play an active role in a realistic, personalized treatment plan that will help them achieve their goals.
